We help our members find new homes with love and caring support. Some of our services include:

- Assisting members in SNF with obtaining identification documents.
- Offering guidance on how the transition program works, and what to expect.
- Assisting members with applying for housing voucher that they can use to obtain an apartment or other form of housing.
- Working step-by-step with the member to ensure needed support services are set up prior to discharge.
- We provide our members with all the necessities they need for when they move into their new home (furniture, toiletries, food, cookware)

The goal of an Independent Living Skills Trainer is to help the individual(s) with:
​
- Financial Obtainment: Basic calculation skills, patience, verbal and written communication, dedication, problem solving & decision making.
- Life Management: Coach individual(s) on life management skills.
- Health & Wellness: Having the ability to advocate for one's self, verbal communication, motivation & self determination.
- Community Integration: Assist/coach individual(s) to become aware of his/her surroundings and getting involved in the community.
