Members who qualify for Health First Colorado services can enter the Transition Coordination program. Once we receive a referral to work with a member, we will come to meet them and perform an assessment to formulate a plan based around their needs. We will help the member apply for a housing voucher, and assist them to find an apartment where they can start a new chapter in their life. Altruistic Transition Agency will also work with the member's case manager (with the county) to ensure that adequate services are set up for when the member moves into their new home.
