"Helping with an Open Heart"
Altruistic Transition Agency was founded in 2018 as part of the Colorado transitions program, which operates through Health First Colorado (Colorado's Medicaid program). Our objective is to assist eligible Medicaid participants with transitioning from skilled nursing facilities (SNF) or rehab institutions, back into the community. We provide support to members in our program every step of the way, and we help guide them as they make life changing decisions.
